Immunizations

Immunizations are key to promoting a healthier community by helping prevent or lessen the severity of certain diseases. Immunizations cost less than disease treatment, are recommended by health care providers, and are required prior to attending school or child care.

Oklahoma law requires that parents of all students of all grade levels attending school or child care facilities, submit acceptable evidence of current, up-to-date immunizations or a completed and signed exemption form. Such evidence is required before the child is allowed to enter or attend school or child care. [Enter link for Oklahoma Immunization Law]

The State of Oklahoma also requires all students entering into the 7th grade receive one Tdap booster for immunization against tetanus, diphtheria, and acellular pertussis. Parents/Guardians are encouraged to contact their child’s healthcare provider to arrange for this immunization and will need to provide their school with a copy of their immunization record showing completion of the Tdap booster prior to starting school.

Per Oklahoma State requirements for school enrollment, a parent or legal guardian shall provide one of the following:

  1. Current, up-to-date immunization records

OR

A completed and signed exemption form

Exemptions

Children with specific medical contraindications to any or all immunizations may apply for an exemption. The children will be allowed to attend school if the medical reason is stated and the statement is signed by a licensed physician.

Children whose parents have objections to immunizations based on religious teachings or personal beliefs may also apply for an exemption.

Process to apply for Immunization Exemptions

  • Print off an exemption form
  • Complete the form in its entirety with signature
  • Turn in completed form to your child’s school to be submitted
  • Completed form will be sent to the Oklahoma Immunization Service for approval
  • If form is not approved, you will be contacted from your child’s school nurse
